MENTAL HEALTH CASE MANAGER
Goochland Powhatan Community Services is seeking a Mental Health Case Manager to provide services to adults with serious mental. This position works closely with the nurse and psychiatric providers within the agency and as well as community providers. These services utilize a team approach and are coordinated within the agency, community partners, and the family to meet the needs of the consumers in supporting independent living. Duties include completing psychosocial needs assessments, developing Individual Service Plans (ISP), monitoring sufficiency of services and goal attainment, collaborating with collateral partners, assisting in medication management, providing education, scheduling appointments, making referrals as needed, documenting in an electronic health record, coordinating services, monitoring medication compliance, providing support with maintaining housing, and meeting individuals face-to-face in the community and their homes. The
Case Manager is responsible for preparing documentation with accuracy and completeness.
Thetypical case load will be 40-50 cases.
The qualified candidate must have good oral and written communication skills and strong interpersonal skills. Qualified applicants must have a high comfort level with technology. GPCS uses Credible, a web-based electronic health record.
Current valid driver’s license and good driving record required. Bachelor’s degree required.
A Bachelor’s or Master’s in a Human Service field is preferred. The candidate must meet the qualifications for Qualified Mental Health Case Manager. One to two years related experience in case management and working with the severe mental health illness population preferably in a community service board or behavioral
health authority.
Salary is commensurate with experience and qualifications, starting at $50,000/year. GPCS offers an excellent benefits package, including an option for free insurance for the employee and participation in the Virginia Retirement System
